Furniture collection booking form

We’re always looking for quality donations of furniture and homeware to stock our growing number of shops in the Thames Valley. The sale of your donations helps raise vital funds so we can care for local people with life-limiting illnesses. To book a collection, please complete the form below. IMPORTANT: in order to comply with government guidelines and ensure social distancing, we currently can only collect or deliver furniture from outside a residency or in a garage. We cannot enter people’s homes. This is to ensure the health and safety of those donating and our staff.

Gift Aid

Yes, I am a UK taxpayer and I would like Thames Hospice to treat all donations I have made for the four years prior to this tax year, today, and all donations I make from the date of this declaration as Gift Aid donations, until I notify you otherwise.

I understand that if I pay less Income Tax and/or Capital Gains Tax than the amount of Gift Aid claimed on all my donations in that tax year it is my responsibility to pay any difference. Gift Aid is reclaimed by the charity from the tax you pay for the current tax year and your home address is needed to identify you as a UK taxpayer.

I confirm Thames Hospice Retail Ltd will act as my agent in selling the goods, from which a 2.5% commission fee + VAT will be deducted from the proceeds of the sale. The net proceeds from the sale of the goods will be donated to Thames Hospice. It will not be necessary to write to me to confirm the donations during the year, provided that the total net proceeds from the sale of the goods so far in the tax year has not exceeded £1,000. Thames Hospice will write to me to seek consent to those funds over and above the initial £1,000 being donated, and will write with updates as further goods are sold. Unsold goods will be disposed of. We reserve the right to terminate the agency agreement at any time. By completing this form you are also confirming you are not acting as a business in selling your goods.
